The average senior adults see 4 types of physicians. Senior adults frequently use both a local and mail order pharmacies, and take many over-the-counter and alternative products. The problems associated with the above create a polypharmacy scenario that can cause many side effects as well as drug/drug and drug/disease state interactions. This program will help define the role of medication management and opportunities to request de-prescribing medications.
